Advance Directives- Need Help

I'm in the process of writing my A/D and I need a little help with it.

I don't want anyone being able to make any decisions for me- especially doctors.

How do I word this? If I'm unable to make any medical decisions, how do I write this to reflect that?

Any advice would be welcome.

I know someone who did a medical care directive on a points system. Like "Start with the number of points equal to 80 minus my age. Subtract X points for each day spent in a coma after the first three days. Subtract Y points each for the failure of lungs, kidneys, or heart. When the number of points is 0 or fewer, apply no medical treatment beyond pain relief." The metrics that were picked were very objective ones which aren't a matter of opinion. Maybe that's the way to address these things if you don't have a particular person in mind to take care of these issues.

My living will states not to pull life support unless permanent brain dead.
