A few months after Dr Michael Kamrava helped Nadya Suleman, who already had six children, get pregnant with eight more, he also implanted another patient with seven embryos.

The unnamed woman in her late 40s, like Miss Suleman, wanted only one more child, but is now five months pregnant with four babies.

She is receiving treatment at Los Angeles County USC Medical Centre, according to multiple sources cited by Saturday's Los Angeles Times.

The woman carrying quads is without medical insurance, usually an indication that she comes from a low income home less likely to be able to cope with the arrival of a large family. She already has three adult children from a previous marriage but wanted one more child with her new husband, who is in his 30s.

The news is likely to reinforce criticism of Dr Kamrava's practice and strengthen calls for better regulation of the fertility industry. His activities in the octuplets case are already under investigation by the California Medical Board and the American Society for Reproductive Medicine amid claims that woman carrying more than two babies face increased medical risks.

The LA Times quoted Dr John Jain, a fertility doctor familiar with the new case, saying: "I do think it is concerning, and dangerous, especially to the mother. She is close to 50. When women get to be that age, our fear is the cardiovascular complications, such as stroke or heart attack. That's how serious this is." Word that the 33-year-old single unemployed Miss Suleman is receiving welfare to care for her 14 children , all conceived through in vitro fertilization, has sparked public anger and led to death threats against both the 33 year old mother of 14 and her representatives.

The firm that has been doing her PR has announced that it is standing down as a result of the barrage of 100 hate emails and voicemails.

Joann Killeen of the Killeen Furtney Group said: "They'd put me in the wood chipper and throw me in the bottom of the ocean and hope I die. We've got her through the worst part of it and now they are putting their venom and anger toward us. We've never had a public reaction to us representing a client pro bono like this, ever." Miss Suleman has now hired an agent called Wes Yoder to represent her who organised press, television and book deals for Bobbi and Kenny McCaughey, the Iowa couple who had the world's first surviving septuplets in Nov 1997.

West Los Angeles police are investigating the death threats.
